Lucknow: A team of specialists at Max Super Speciality Hospital, Lucknow, has successfully treated a rare and challenging case of hepatolithiasis, offering a fresh lease of life to a 27-year-old patient from Delhi.
The patient, Rishabh Singh, had been suffering for months with recurring fever, jaundice, abdominal pain and significant weight loss. Detailed investigations at Max Hospital revealed that he was battling Recurrent Pyogenic Cholangitis, a chronic condition marked by repeated infections and inflammation of the bile ducts. This had led to the formation of multiple stones deep within the liver — a condition uncommon at his age and notoriously difficult to treat.
Despite undergoing a prior medical procedure at another hospital, his symptoms persisted and his health continued to deteriorate before he sought care in Lucknow.
Explaining the complexity of the case, Dr. Waliullah Siddiqui, Director – Hepato-Pancreato-Biliary & Liver Transplant Surgery at Max Hospital, said the combination of active infection and multiple intrahepatic stones required a highly coordinated, multidisciplinary plan.
“When Rishabh came to us, he was already facing repeated infections and rapid weight loss. Our evaluation confirmed multiple stones in the left lobe of the liver along with damaged bile ducts, making this a technically demanding case,” he noted.

The surgical team proceeded with an advanced liver surgery in which the diseased left portion of the liver was removed. The damaged extrahepatic bile duct was also excised, and a new connection was created between the healthy right liver duct and the intestine to restore normal bile flow.
The operation was successful, and the patient made a steady recovery. He was discharged on the eighth postoperative day, and his one-month follow-up showed complete resolution of symptoms and a return to normal health indicators.
